A declaration of non-receipt (affidavit) is a form with which you officially confirm that you have not received a parcel.
What do you confirm with this declaration?
With your signature, you confirm that:
- The parcel did not arrive to you
- It was not delivered to neighbors or other persons
- The information you provide is truthful
Why is the declaration of non-receipt required?
The declaration is required so that:
- the shipping provider (e.g. DHL) can investigate the case
- a possible loss can be officially confirmed
- we as the retailer can arrange a replacement shipment or refund
👉 Without this declaration, processing is often not possible.
What information must be completed?
- Name and address
- Tracking number
- Sender
- Contents of the parcel (items/value of goods)
- Selection of the appropriate declaration (e.g. “not received”)
- Date and signature
